TOC Clinical Pharmacist

Summary

Supports the mission of the hospital by aiming to reduce hospital readmissions through patient discharge optimization and follow-up. Performs clinical duties and engages in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourages provider follow-up and continuity of care following hospital discharge.

Minimum Qualifications

Experience is PGY‑1 residency; PGY‑2 residency.

Education

• Bachelors Degree
• Doctoral Degree
• Pharmacy specialization

Education Equivalent Experience

• Bachelors – Pharmacy (required)
• Doctorate – Pharmacy (preferred)

Credentials

• Pharmacist (essential)
• Pharmacist – State of Florida (equivalent required)
• ASHP (Pharmacists) (preferred)
