Travel PT Home Health jobs in Westchester, NY are available for 13-week assignments, working Monday through Friday 8-hour days with 40 hours per week. You must have a current New York license and experience with the geriatric population. Home Health experience is preferred, and you will use Homecare Home Base for documentation.Your responsibilities include assessing and treating patients in their homes, developing individualized care plans, educating patients and families, evaluating home safety, and coordinating care with other providers.
