Team Lead - One Link (Mental Health and Addictions Coordinated Access)

The Team Lead is responsible for the day-to-day clinical operation and patient flow of the Mental Health and Addictions Hub program including:

• Acting as the point of contact for referral flow to the program, liaising with referral sources and partner mental health and addiction services.
• Acting as a clinical resource for problem solving complex cases and identifying service coordination options in collaboration with health service partners.
• Supporting the clerical staff with prioritization of transcription, patient visit bookings, re‑scheduling and workflow.
• Collaborating with internal and external stakeholders to ensure ongoing evaluation and continued streamlining of coordinated access and updating processes as needed to facilitate timely access to care.
• Supporting quality improvements and evidence-based practice for all members of the team.
