Transform the lives of seniors as a full-time Occupational Therapist with Senior Persons Living Connected (SPLC). This position focuses on providing innovative care to enhance the daily living of older adults.
As a member of the GAIN Community Team, you will be responsible for comprehensive assessments that inform interventions for seniors facing a variety of aging-related challenges. Your in-depth assessments and collaborative care planning will be vital for supporting their needs and improving their quality of life. At SPLC, you can expect to work in a nurturing environment that prioritizes professional development and community engagement.
Key Responsibilities:
• Participate in Comprehensive Geriatric Assessments as a team member
• Complete triage for new patient referrals
• Collect and analyze clinical data thoroughly
• Deliver consultation services to enhance daily living
• Establish collaborative care plans with the inter-professional team
