The Registered Nurse (RN) – Case Manager, Seniors Mental Health (SMH) Outreach, will work as part of an inter‑professional specialty SMH Outreach team providing comprehensive assessment; consultation; risk management; and support for older adults living in the community with complex cognitive‑mental health needs, often with multiple physical co‑morbidities. The RN Case Manager will assist with the development of an integrated delivery of service to enhance the quality of life for older adults with complex cognitive‑mental health needs and/or behavioural disturbance through partnerships with various community services supporting seniors across the system of care. The RN Case Manager will coordinate and integrate services and resources to assist the older adult with ageing at home and promote system integration.
