As a vital member of the 4NW Surge team, you will work under the direction of the Patient Care Manager. Your role will involve collaborating with Registered Nurses (RNs) and Personal Support Workers (PSWs) while delivering care according to established plans. Your expertise in geriatric and acute medicine will help support a diverse patient population requiring transitional care.
Key Responsibilities:
• Deliver nursing care based on pre‑set care plans
• Assess patient needs and monitor health outcomes
• Collaborate with RN and healthcare team members
• Provide support during patient transitions and discharges
• Uphold health and safety standards consistently
