Overview

This is an exciting opportunity to work with our outpatient primary care team that is expanding to provide increased access to patients who are not attached to primary care providers. You will work closely with our inter-disciplinary team to support the attachment of patients to our clinic. You will have opportunity to support the development of services as part of our expansion.

Responsibilities

• Assess patient health care needs (physical, emotional, psychological, and spiritual) to identify appropriate nursing interventions


• Analyze findings of health assessments


• Interpret patient health records


• Monitor, record and report symptoms and changes in patients’ conditions


• Collect data through therapeutic relationships with patients
