Primary Care Nurse Practitioner - Stockton / Modesto / Amador

Role Overview

We're seeking Primary Care NPs to provide on‑site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.

What You'll Do

• Routine & urgent assessments; chronic disease management
• Post‑discharge follow‑ups, medication reconciliation, labs/diagnostics
• Coordinate with facility staff, specialists, families/POAs
• Same‑day documentation; CMS/regulatory compliance

Schedule & Setting

Full‑time position with a structured weekday schedule. Providers deliver on‑site care across Stockton, Modesto, and Amador with strategically clustered routes, so you spend more time with patients and less time on the road.

Requirements

• Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
• Geriatrics or facility‑based experience preferred; EMR proficiency
