Location: Colorado (Hybrid On-Site/Remote)
Schedule: Monday-Friday No Nights or Weekends

Position Overview:
A mission-driven, interdisciplinary care program is seeking a Board-Certified/Board-Eligible MD or DO with a background in Internal Medicine, Geriatrics, or Family Medicine to join its Skilled Nursing Facility (SNF) Transitions Team, with a strong focus on geriatric and palliative care. This is a unique opportunity for a physician with hospitalist or SNF experience seeking a rewarding role with excellent work-life balance.
Key Responsibilities:
Provide compassionate, high-quality subacute and palliative care for medically complex older adults in contracted SNFs
Manage a census of 20-25 patients, with 5 average admissions/discharges per week
Conduct bedside rounds 2-4 times weekly with flexible hybrid work options...
