Role and Responsibilities
A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home , following physician’s orders.
Essential Functions
+ Evaluate patient using diagnostic muscle, nerve, joint , and functional ability tests and measures.
+ Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.
+ Recommend modification of plan of care to physician, as needed.
+ Document clinical notes on the date of the visit into medical record and keep all records and summaries up - to - date ...
