Role Overview

Our valued partners are seeking a Patient Outreach and Enrollment Specialist (POES) to provide critical healthcare enrollment assistance and patient navigation services across a growing network of state‑of‑the‑art health centers serving California’s Central Valley. Reporting directly to the Patient Outreach and Enrollment Supervisor, the ideal candidate will possess a High School Diploma or GED and at least one year of experience in health education, public health, community outreach, or a related healthcare field, preferably within a Federally Qualified Health Center (FQHC).
