Key Responsibilities:
• Assist patients during hospital discharge processes
• Collaborate with others to prevent re-hospitalizations
• Empower clients for health success at home
Requirements:
• Must be registered with the College of Dietitians of Ontario
• Strong communication skills for teamwork
• Experience with tube feeding is advantageous
• Critical judgment and empathetic understanding necessary
• Ability to travel within the community
