Job Description
Make a difference in mental health as a Program Coordinator focused on housing solutions. Engage with clients, manage referrals, and conduct assessments in the Aurora area.
As a Program Coordinator for the Housing First Program, you will work alongside the Manager to coordinate essential mental health and financial support services. The role involves assisting with community outreach, providing client supports, and ensuring accurate documentation of services provided. You'll also play a key role in liaising with third parties to enhance client treatment plans.
Key Responsibilities:
• Coordinate incoming client referrals and assessments
• Lead planning and execution of community events
• Facilitate assignment of clients to staff as needed
• Document client activities in CRMS and HIFIS databases
• Monitor financial requests and track service spending
