- Location: Phoenix, AZ (with 2 days/month in Casa Grande ASC)
- Schedule: Clinic + procedural mix (1-2 days/week in ASC)
- Compensation: $450,000 guaranteed base
- Bonus Structure:40% of office collections
- 25% of ASC collections (after implant costs)
- Benefits: Medical, Dental, Vision; 4 weeks PTO/Sick Time; $2,000 CME stipend; malpractice and license coverage
- Sign-On / Relocation Bonus: Available; enhanced packages for physicians bringing an existing patient panel or practice
- Work Environment: Established outpatient pain management practice with access to ASC
