Join ParaMed Home Health Care as a Home Care Registered Practical Nurse, providing compassionate care to patients of all ages. This full-time role allows for autonomy while supporting patients' palliative care needs in their homes.
In this community Shift Nursing position, you will conduct scheduled patient visits, assess individualized needs, and execute care plans with a collaborative approach. Your ability to build trust and rapport will enhance long-term therapeutic relationships while you maintain accurate patient records during your visits.
Key Responsibilities:
• Assess individualized patient needs using appropriate techniques
• Execute care plans and monitor patient progress
• Build rapport and trust with patients through engagement
• Maintain up-to-date patient records for all encounters
Requirements:
• Registered with the CNO as an RPN in good standing
• Valid driver’s license and access to a vehicle
• Experience in community care is an asset
