Health Navigator

Application Information

Reporting to the Director of Acute and Clinical Services, or designate, the Health Navigator serves as a central resource to support patients presenting to the Emergency Department and those admitted to the ANHH who require complex discharge planning and care coordination.

Duties and Responsibilities

• Utilize a holistic model to assess patient and family medical, social, cultural, and spiritual needs.
• Advocate for patients and families through their care journey with navigation, resource connection, and follow-up case planning.
• Promote culturally safe and trauma-informed care practices by utilizing cultural and traditional supports including First Nations Cultural Practitioners.
• Lead and coordinate complex discharge planning and family meetings for patients admitted in the hospital, including liaising with community-based services.
