Join Surrey Memorial Hospital as a Clinical Pharmacy Specialist focused on Antimicrobial Stewardship in Surrey, BC. This full-time position emphasizes evidence-based pharmaceutical care and enhancing patient outcomes.
Collaborate with Fraser Health and other premier health organizations to implement and assess the Antimicrobial Stewardship Program. You will provide clinical direction, evaluate drug therapies, and develop educational materials. Your leadership will ensure that healthcare professionals have the necessary knowledge for optimal patient outcomes.
Key Responsibilities:
• Plan and assess AMS programs at the facility level
• Identify areas for drug usage evaluation based on historical data
• Monitor antimicrobial drug-utilization records
• Develop and implement key AMS documents and guidelines
• Conduct evaluations to support medication management decisions
