Job Description
Support seniors in need as a Client Support Worker at WoodGreen in Toronto. This full-time role focuses on emotional and practical assistance for seniors facing homelessness.
In the Cedarvale Housing Program, you will engage directly with seniors, offering vital support tailored to individual care plans. You'll facilitate housing stability and encourage community participation while collaborating with external services to provide comprehensive assistance.
Key Responsibilities:
• Build positive relationships with program seniors
• Assist clients in completing forms and accessing resources
• Participate in intake, screening, and onboarding processes
• Monitor progress and adjust care plans effectively
• Respond to crises with de-escalation strategies
Requirements:
• Diploma in Social Service Work or similar field
• 2+ years experience with older adults in need
• Strong interpersonal and conflict resolution capabilities
• Ability to lift up to 40 lbs
