Fraser Health invites you to join an innovative primary health care team located in one of Canada’s fastest growing and culturally diverse cities. The Urgent and Primary Care Centre (UPCC) offers full‑time, part‑time, and locum opportunities. Our UPCC opened in the Newton neighbourhood of Surrey, BC in May 2020 and operates in a team‑based care model that includes physicians, nurse practitioners, nurses, social workers, clinical counsellors, allied health professionals and administrative support.
Responsibilities
- Participate as a full and active member in the delivery of team‑based primary‑care services through the clinic as part of a longitudinal care team.
- Function as an essential member of the clinical care team and manage a panel of patients.
- Provide patient‑centred and trauma‑informed care, serving the vulnerable and medically complex in the area.
