Job Description

Build and maintain strong relationships with hospitals, community organizations, and key stakeholders to identify and advance partnership opportunities aligned with organizational and health system priorities.

Support the planning and implementation of strategic initiatives (e.g., Hospital-to-Home programs, integrated care pathways) in collaboration with operational teams, corporate functions, and external partners.

Facilitate cross-functional collaboration to ensure initiatives remain aligned, risks and barriers are identified, and drive progress towards overall objectives.

Support governance structures and stakeholder engagement activities by coordinating collaborative forums and ensuring timely follow-through on decisions.
