This role is essential in managing high-risk patient populations and contributing to quality improvement initiatives.
Key Responsibilities:
• Conduct patient medication assessments to resolve issues
• Support specialized care for memory and geriatric patients
• Optimize medication use in respiratory programs
• Offer evidence-based drug recommendations to the team
