Through trusted relationships and community-based support, we partner with health plans and local organizations to help members navigate healthcare, stay engaged in care, and live healthier lives.
We are launching a new clinical pilot that integrates physicians, care navigators, and community support teams to create a more coordinated and human approach to care.
The Role: We are looking for a Clinical Operations Associate to be the primary point of contact and logistical driver for our members. In this role, you own the Everything Surrounding Care space—ensuring that from the moment a referral arrives, the patient has everything they need to successfully meet with their clinician.
