Enhance senior care as a part-time Clinical Consultant Pharmacist at CareRx in the Waterloo Region. Drive improvements in medication management and clinical outcomes.
As a Clinical Consultant Pharmacist, your primary role will be to support healthcare teams within long-term care and retirement communities. You will ensure continuity of care and foster relationships that benefit patient health. Your tasks will include medication reviews and educating both staff and residents on effective health practices, all while adhering to CareRx policies and provincial regulations.
Key Responsibilities:
• Foster partnerships with care community leaders
• Complete on-site medication reviews
• Conduct health education sessions as needed
• Ensure compliance with pharmacy standards
