Reporting to the Director of INTEGRAcare East, this full-time role offers you the chance to lead care management and support clients' independence and joy. You will develop care plans, train new staff, and manage the schedules of caregivers to foster continuity in care.
Key Responsibilities:
• Develop and implement dynamic care plans for clients
• Schedule Caregivers focusing on care consistency
• Facilitate training and orientation for new hires
• Be part of the on-call team for immediate support
• Participate in team-building events to strengthen collaboration
Requirements:
• Must have valid CNO Registration
• Preferred 1-2 years nursing experience
• Bilingual in English and French
• Excellent attention to detail and communication skills
