Job Description
Lead a dedicated care team at Acclaim Health as a Care Manager for Personal Support Workers (PSWs) in Oakville. This full-time position features a Monday to Friday schedule with evening flexibility.
As a Care Manager, you will supervise three PSWs, guiding their efforts to deliver essential care in the community. With a focus on patient needs, you will assess and implement collaborative care plans ensuring the highest standards of patient-centered care. Your leadership will enhance the support environment for staff and ensure compliance with quality initiatives.
Key Responsibilities:
• Assess and monitor patient care plans regularly
