Care Coordinator

The Opportunity

The Care Coordinator is part of our amazing Amplar Preventative Health Team.

In this role you will work through a variety of service delivery modes –video and telephonic delivery or digital programs to support people with complex and chronic care needs in the community.

Drawing on your holistic knowledge and background in health, specifically chronic health management, you will support health literacy and empower participants to self-manage their health. You will utilise a coordinated and integrated approach, facilitating health behaviour change and care coordination in the delivery of health coaching.

This is a unique work from home role that allows you to be located anywhere in Australia. We can offer permanent and fixed term opportunities with full or part-time hours (minimum 0.6FTE). It is required to have suitable office area from which to work and reliable internet connection. ...
