Job Description
Make a difference as an Aging at Home Program Coordinator with the Métis Nation of Ontario in Peterborough. This role is dedicated to facilitating independent living for seniors through community support.
The Aging at Home Coordinator will provide vital services to seniors 55+, managing friendly visits, coordinating support assessments, and ensuring they receive the necessary assistance from contracted services. Responsibilities also include tracking service delivery and maintaining client confidentiality while adhering to privacy regulations.
Key Responsibilities:
• Implement Aging at Home program according to Métis Nation values
• Conduct service support assessments for clients
• Coordinate lawn care and snow removal services
• Assist clients in accessing transportation for appointments
• Document services using OHATS and Excel
