Job Description

Purpose:
Works as a member of the multidisciplinary team to assess, plan, develop, coordinate and provide treatment, rehabilitation, crisis intervention and support services to program clients with severe and persistent mental illness, with complexities including concurrent disorders and physical health complications under the clinical supervision of the Program coordinator and psychiatrist.
Duties:
Provide and coordinate treatment for an assigned group of clients.
Provide counseling and therapy to assist clients in developing skills to deal with and resolve their social and personal problems.
Plan programs of assistance for clients including referral to agencies that provide financial assistance, legal aid, housing, medical treatment and other services.
Serve as a member on interdisciplinary teams of professionals working with client groups.
